This consent form explains what data is shared through Moovama, who can access it, and your rights regarding this information. Both the elderly user (data sharer) and family member (data receiver) must understand and agree to these terms.
For Elderly Users (Data Sharers)
What You're Consenting To
By connecting your wearable device and designating family members, you consent to:
- Moovama accessing your wearable device activity data
- Sharing this data with family members you designate
- Sending periodic SMS summaries to those family members
- Storing your data for up to 90 days
What Data Is Shared
- Daily step count
- Average heart rate
- Sleep duration
- Active minutes
- General activity trends
What Is NOT Shared
- Your exact location or GPS data
- Detailed medical information
- Personal messages or communications
- Financial information
- Information beyond basic activity metrics
Your Rights
You have the right to:
- Revoke consent at any time by disconnecting your device or deleting your account
- Choose which family members receive your data
- View what data has been shared through your dashboard
- Request deletion of all your data
- Change your mind without penalty or explanation
For Family Members (Data Receivers)
What You're Agreeing To
By signing up to receive updates, you agree to:
- Receive SMS text messages with activity summaries
- Respect the privacy of the shared information
- Understand this is NOT medical information
- Not make medical decisions based solely on this data
- Pay the monthly subscription fee ($6/month)
Your Responsibilities
- Confirm consent: Ensure your loved one has actually consented to share their data
- Respect privacy: Do not share this information with others without permission
- Seek medical advice: Contact healthcare professionals for health concerns
- Emergency response: Call 911 for emergencies, not Moovama
Your Rights
You have the right to:
- Unsubscribe at any time by replying STOP or canceling your account
- Change notification preferences (frequency, timing)
- View activity history through the dashboard
- Request your data be deleted

- Moovama is NOT a medical device or medical service
- We do NOT provide medical advice, diagnoses, or treatment
- Activity data may be inaccurate, incomplete, or delayed
- Do NOT use Moovama for medical emergencies
- Always consult healthcare professionals for medical concerns
Service Limitations
- SMS messages may be delayed by carriers
- Wearable devices may not sync immediately
- Internet connectivity is required for data transmission
- Service interruptions may occur for maintenance
- Battery life affects wearable device functionality
How to Revoke Consent
Either party can revoke consent immediately by:
For Elderly Users:
- Log into your account
- Go to Settings → Connections
- Click "Disconnect Device"

For Family Members:
- Reply STOP to any SMS message
- Log into your account and cancel

Consent Documentation
During signup, both parties will be asked to explicitly check boxes confirming:
